I keep seeing the same pattern in teams: the work is real, the people are smart, but the process quietly rewards vague language. “Low risk.” “We can roll back.” “Approved in chat.” Then rollout night arrives and those sentences turn into work. Fast.

Hard rule: if success is defined as “we shipped,” you don’t have success criteria yet. You have a deployment.

Example A: standard change (model-based, no CAB time)

A repeatable patch with a known procedure and a stable runbook. This should be pre-approved as a standard change so CAB does not waste time on it.

  • Runbook is the approval artifact.

  • PIR only if something unexpected happens.

Example B: high-risk normal change (conditional approval)

A database migration with real blast radius. CAB approval can be reasonable, but it should be conditional on rollback readiness and monitoring gates.

Do all changes need CAB?
